How to break and build habits to lose weight

It's easy to say "diet and exercise" is all we need for successful weight management. Sure, a well-balanced diet and consistent exercise routine will help us live at a healthier weight, but how do we go about achieving long-term success in these areas?

The reality of the matter is that our subconscious habits play a massive role in our decisions and lifestyle choices such as our dietary and exercise regimens.
