Privacy Policy


The following text outlines the ‘Privacy Policy’ set forth by LEAP HEALTH CORPORATION  ("LEAP,", “we” “our” or "us"). We will use this Privacy Policy in an effort to protect the information you ("User" or "you") provide to us while using or accessing our website and its subdomains, and our associated mobile app (collectively, the “Site”), and/or while utilizing the services offered by LEAP, that includes but is not limited to, in-person and/or online consultations, appointments, sessions, programs, and treatments (collectively, the “Services”). This includes your contact information, personal information you submit during the rendering of Services and personal health information (your “PHI”), if the Services that are rendered are covered by the Health Insurance Portability and Accountability Act of 1996 (HIPPA). 

By accessing, using or engaging in the Site or Services, you understand that you are subject to the Terms of Use which includes this Privacy Policy; meaning you understand and accept all the applicable practices, policies and procedures outlined in the Terms of Use document. Please refer to the Terms of Use document/page for reference to any terms we may use that is not definitively defined here (e.g. “Content”, “Providers”). 

Utilization of our Site and Services are completely optional. However, by utilizing, accessing or engaging in the Site and/or Services, you are agreeing that any personal information you choose to disclose and any website or network-driven information collected as you browse the Site or engage in Services may be received and stored by LEAP and other third-party analytic platforms in any way we deem reasonable. Examples of information you disclose may include but is not limited to, email, phone numbers, credit card information, addresses, and other personal details. This information will be needed by LEAP to ensure accurate billing, customer identification, contact information, and so on. Examples of network-driven information is the use of "cookies.";  an element of data that a website can send to your browser, which may then be stored used to provide you with more specific information at a later time. You acknowledge and agree that LEAP is the sole owner of the information collected on the Site, through our Services and/or through correspondence with you (including emails, texts and other messages).

You understand that some Services are NOT medically necessary and some Services and are NOT rendered by a licensed medical professional. When this is the case, your PHI will not be protected as outlined by HIPPA.​ Services included in this non-protected category of Services are all online services with LEAP. Any information that you submit or that LEAP collects during online services shall NOT be considered PHI and it will not be protected to HIPPA standards.

LEAP will not trade, rent or sell the information you provide to unaffiliated parties. Any personal information we collect from you will be for the purposes of performing or improving our Services, Site, Content, or business functions. LEAP reserves the right to share any information we collect to Providers, contractors, affiliates or agents (outside persons/businesses we contract to perform tasks on LEAP’s behalf). Information may or may not be de-identified, based off LEAP’s judgment.

Additionally, LEAP reserves the right to release or share (with proper authorization as permitted under HIPPA if applicable due to Services received by you), any information you provide or your PHI with other health professionals, business affiliates (including but not limited to third-party billing or payment companies, consultants or agents), insurance companies or other business/individuals related to LEAP business management any information collected from you when we believe it is appropriate to comply with the law, other agreements, or to protect the rights, property, or safety of LEAP, our Providers, other Users or the general public.

We will only share information or PHI you provided us with a family member, guardian or other individuals if specifically request for us to do so. For Users under the age of 18, a parent or legal guardian does not need the User's consent to access information. LEAP reserves the right to share your information in the event of an emergency or for the benefit of the public good, such as infection control or a product recall. 

LEAP may choose to buy or sell assets, and may share and/or transfer customer information in connection with the evaluation of and entry into such transactions. Additionally, if LEAP (or our assets) are acquired or merged, or if we go out of business, enter bankruptcy, or go through some other change of control, your information could be one of the assets transferred to or acquired by a third party.

LEAP and Providers reserve the right to communicate with you for any reason we deem necessary. You have the right to opt-out of some forms of communication such as fundraising and promotional emails. Please email us if there are any forms of communication you would like to be excluded from. 

You reserve the right to be notified should a breach of your information occur. You also reserve the right to file a complaint if you feel like your rights were violated. LEAP will never retaliate against you for filing a complaint. 

You understand and agree that LEAP reserves the right to modify, adapt and otherwise change this Privacy Policy from time to time, without obligation to notify you. You understand that it is your responsibility to stay current on our Terms and policies, including the Privacy Policy. If you utilize, access or engage in the Site or Services after changes to the Privacy Policy have been made, you are agreeing to the updated terms.

We will make reasonable accommodations to secure your personal information and PHI. You understand that you as the User must also make reasonable efforts to protect your information such as securing your emails, accounts, passwords, and devices. Unfortunately, LEAP cannot guarantee the complete, total, and sustained security of your information. “Hackers”, personnel errors, technology failures, and other factors may compromise the security of your information. If we discover a breach in security has been made, you will be notified per applicable state and federal laws.
